4CD Vaccine Requirement Exception Request Information

Students and employees may request a medical or religious vaccine exception if one of the following situations apply:

• Medical excuse from receiving COVID-19 vaccine due to medical conditions or precaution (requires medical documentation)

• Disability or pregnancy (requires medical documentation)

• Religious objection based on a person’s sincerely held beliefs, practice or observance

Students:

WEEKLY TESTING (for those with approved exceptions)

All students and employees who have received an approved exception must test themselves on a weekly basis. Free COVID-19 antigen tests are provided at all District locations. Students and employees may pick up a free test on a weekly basis at the following locations:

• Contra Costa College — Student Life Office, Student & Administration Building, Room 104

• Diablo Valley College (Pleasant Hill Campus) — Student Life Office, Student Union Building

• Diablo Valley College (San Ramon Campus) — Division Office, West Building, Room 132

• Los Medanos College (Pittsburg Campus) — Student Life Office, Student Union Building (2nd Floor)

• Los Medanos College (Brentwood Center) — Admissions & Records Counter, Student Services Lobby in the Administration Building

• District Office — Front Desk

All students and employees are expected to follow CDC guidelines if they test positive for COVID-19.
