4CD Vaccine Requirement

As part of the Contra Costa Community College District's ongoing commitment to keep our communities safe, students must submit proof of vaccination (or have an approved exemption) to register for in-person classes or access in person services .

All employees, regardless of primary work location, are required to submit proof of vaccination or have an approved exemption.

4CD offers medical and religious exceptions for eligible students and employees.

SUBMIT PROOF OF VACCINATION

Accepted forms of vaccine documentation include:

  1. A photo or screenshot of an Official California Digital COVID-19 Vaccination Record

  2. A photo of an original vaccination card showing the date(s) and types of vaccination(s) received

  3. A photo, JPG or PNG file (PDFs not accepted) of a form from a healthcare provider that documents the date(s) and types of vaccination(s) received

Action: Open the InSite app and select the Vaccine Upload tile. Follow these steps to upload your document:

  1. Click the Vaccine Upload tile to open the Required Documents page.

  2. Click the manage button on your required document to upload your proof of vaccine.

  3. Once uploaded, your document will be in Pending status until reviewed and approved.

  4. You will be notified when your document is approved, and then you'll be able to register for classes.

*Students and employees who do not have a smart phone may login to InSite here and follow the steps above to submit their proof of vaccination.

VISITORS

It is strongly recommended that visitors become fully vaccinated prior to entering district facilities.
