4CD Vaccine Requirement

To register for spring 2022 in-person classes or to utilize in-person services (beginning January 3, 2022), students must provide proof of vaccination or receive an approved exception by November 1, 2021. Students meeting this deadline will receive a $150 incentive for helping to keep our communities safe.

Students who do not provide proof of vaccination or receive an approved exception by November 1, 2021, will have a hold placed on their record that will restrict them from registering for in-person classes. Students will not be able to register for in-person classes until they become fully vaccinated or receive an approved exception.

Students who are attending only online classes for the 2021-2022 school year, and who will not be visiting campus at all, may choose not to upload their vaccine information.

All students, regardless of vaccination status, may continue taking in-person classes and utilizing in-person services during the fall semester. No student will be removed or dropped from a class in fall 2021 for not being vaccinated.

All employees, regardless of primary work location, are subject to the vaccine requirement, and must submit proof of vaccination or receive an approved exception by November 1, 2021.

Students and employees who are fully vaccinated can upload their vaccine documentation to 4CD Safe via the link below. A person is considered “fully” vaccinated when two weeks have passed since they completed a COVID-19 vaccine series (e.g., one dose of Johnson & Johnson, or both doses of a Moderna or Pfizer vaccine).

  • Submit Proof of Vaccination ( NOTE: Students must use their InSite email address and employees must use their full @email.4cd.edu email address. Acceptable file formats include .JPG and .PNG.)

Accepted forms of vaccine documentation:

  1. An image or screenshot of an Official California Digital COVID-19 Vaccination Record

  2. An original vaccination card showing the date(s) and types of vaccination(s) received

  3. A form from a healthcare provider that documents the date(s) and types of vaccination(s) received

Students and employees may request a medical or religious vaccine exception if one of the following situations apply:

  • Medical excuse from receiving COVID-19 vaccine due to medical conditions or precaution (requires medical documentation)

  • Disability or pregnancy (requires medical documentation)

  • Religious objection based on a person’s sincerely held beliefs, practice or observance

Beginning November 1, 2021, students and employees who receive an approved exception will be required to utilize free testing services on a weekly basis.

Students can access exception request forms using the following links:

The employee exception request forms will be available soon.

All employees and students are encouraged to use 4CD Safe to complete a daily self-assessment for COVID-19 symptoms prior to coming to campus.

Beginning January 3, 2022, the daily health assessments and the green ‘symptom-free’ passes provided by 4CD Safe will be required for all employees and students to be on campus. To log in, students should use their InSite email address and employees should use their full @email.4cd.edu email address.

Visitors planning to come to campus should be fully vaccinated and are required to complete a health assessment prior to visiting any District facility.

To protect the health and safety of students, employees, and members of the community, the Contra Costa Community College District (4CD) requires COVID-19 vaccinations for all employees and for all students taking in-person classes or who need to come to campus for any reason (to access tutoring, student services, the Library, etc.). See 9/8/21 Governing Board Vaccine Requirement Resolution.
